JOB SUMMARY
The Director of Care Management leads a team that supports health centers and their patients in accessing primary care, addressing social needs, and following up after hospitalizations. The Director is responsible for hiring, training and supervising a community health worker team, as well as additional team members, who may include clinically licensed team members in the future; developing and implementing processes to achieve optimal outcomes; and serving as the primary liaison with health center staff and other external stakeholders, including payers. The Director works closely with the Senior Director of Population Health to develop and manage the program budget and determine how revenue will be expended. The Director also serves as an organizational resource for other staff and programs related to community health worker training and supervision, value based payment programs, and Medicaid managed care and is part of the Population Health Leadership Team.
JOB SPECIFICATIONS
Responsibilities/Duties:
- Direct a centralized care management and quality improvement program, including developing and refining workflows, optimizing performance, and leading all decision-making regarding the program.
- Hire, train and supervise team members.
- Mentor Community Health Workers and support them in professional development, including pursuing and maintaining certification.
- Manage program data. Use patient/member level data accessed through multiple applications and from multiple sources to establish work priorities, assign tasks to staff, develop reports, and engage with health centers and Managed Care Organizations regarding performance.
- Communicate regularly with health centers, Managed Care Organizations, and other stakeholders regarding performance and patients.
- Manage program budget.
- Continuously assess and improve processes, including evaluating and adding new technology tools as needed and as permitted by budget.
- Work with Senior Director - Population Health to seek out new contracting opportunities and conceptualize and implement portfolio expansion.
- Serve as an internal subject matter expert on community health worker supervision, social needs resources, value-based payment, and care management with other HFP programs.
- Ensure that staff safeguard patient privacy and confidentiality according to HIPAA rule.
- Provide direct outreach and care management services to patients, as needed.
Qualifications
Education:
- RN, LPN, LSW, LCSW or similar licensure strongly preferred.
- Bachelor’s degree in a health or human services field, or equivalent work experience.
Qualifications/Skills:
- At least 5 years experience providing and/or overseeing care management services to patients with significant social needs and chronic physical and/or mental health conditions.
- At least 5 years experience with staff supervision, including supervision of community health workers or similar staff.
- Understanding of managed care and value based payment arrangements.
- Community health center and/or Medicaid program experience is desired, but not required.
- Ability to work independently, flexibility, and adaptability.
- Ability to communicate effectively with a broad range of external partners and engage them in working toward joint objectives.
- Ability to manage data and utilize it to support the program, including proficiency in Google Sheets/Excel. Ability to learn new applications quickly and to provide input into technology decisions.
- Familiarity with Electronic Health Record and Population Health Management systems.
- Excellent written and verbal communication skills.
Position Type and Work Schedule: This is a full-time, exempt position.
Work Environment: Standard office setting, plus regularly scheduled meetings, trainings, etc. with participating health centers. Health Federation’s hybrid work policy allows work from home two days per week.
Physical Demands: Able to sit and work at a computer keyboard for extended periods of time. Able to lift and move up to 15 pounds.
Travel: Must be able to get around effectively within Philadelphia and surrounding counties, either on public transportation and/or in own vehicle. Limited travel beyond surrounding counties will be required, as well.
