Centering empowers patients, strengthens patient-provider relationships, and builds communities through three main components: health assessment, interactive learning, and community building. We are driven to create a future where the risk of preterm birth, the inequities of Black women dying from pregnancy-related causes, and the disparities in early childhood are significantly reduced - so that every child has the potential to enjoy future life chances, social and economic opportunities, and overall well-being. With over two decades of experience as the go-to resource for group care, we’ve developed and sustained the Centering model in nearly 500 practice sites and some of the largest health systems in the world.
Vaccination Requirements
As a condition of employment, each candidate selected for employment will be required to have received at least one dose of a COVID-19 vaccination prior to the selected candidate’s start date with the Company and be fully vaccinated (as that term may be defined by the CDC) within 30 days from that date, unless approved for an exemption. Proof of at least one dose of a COVID-19 vaccination will be required prior to the scheduled start date, and proof of full vaccination will be required no later than 30 days after the start date. Position Summary
The New Jersey Centering Program Manager is primarily responsible for supporting the expansion, implementation, and sustainability of Centering at healthcare practices within New Jersey focusing on high risk populations, addressing health disparities, and reducing infant mortality. They are comfortable engaging with leadership at multiple levels within clinical sites to ensure that the system changes required to meet CHI’s quality and sustainability expectations are achieved. In collaboration with key stakeholders, locally based payers, and policy makers this position will lead sustainability efforts within New Jersey.
There will be extensive travel within the state of New Jersey.

This position reports to the Senior Director of Implementation

NJ Centering Program Manager Responsibilities
Centering Sustainability and Implementation Support
- Provide consultation and technical assistance to existing Centering practice sites to ensure ongoing quality and sustainability of practice; help problem-solve organizational challenges
- Maintain a portfolio of Centering Implementation Plans and guide new Centering practice sites through the implementation process and systems change work required to launch Centering group care
- Prepare practices for CHI’s accreditation process and the long term sustainability of the Centering model
- Ensure that practices are meeting performance expectations, milestones, and timelines established in CHI’s service protocols
- Provide guidance remotely and on-site for the system redesign of clinical practice settings to successfully adopt and maintain Centering group care models, including but not limited to physical modifications, patient flow, staffing, patient enrollment, and evaluation
- Be an active and ‘visible’ resource to assigned practice sites and be readily available and responsive to all communication in a timely manner
- Understand the site’s demographics, unique circumstances, comparable settings, potential alliances, and regional activity to support Centering implementation
- Identify resistance and performance gaps that are obstacles to successful implementation and sustainability, and work with the practice site to develop corrective measures
- Perform accreditation assessments through documentation review and on-site visits to verify model fidelity and sustainability of the Centering model of care
- Maintain a portfolio of client sites in specific regions including, but not limited to, providing sustainability support, leveraging local opportunities for connection/education around Centering, and building strong relationships with clients in the portfolio
- Maintain clear and timely site correspondence documentation and interdepartmental communication
- Provide timely, accurate, reliable communication including activity and expense reports

Community Engagement
- Organize and convene quarterly Consortium meetings and/or Community of Practice to bring Centering care teams from various practice sites together to share and problem-solve
- Engage with payers and key community stakeholders to explore innovative payment models that support the expansion and sustainability of Centering
- Function as a key member of the project team with The Burke Foundation and CHI
- Educate and collaborate with local stakeholders and policymakers in New Jersey on the Centering model, the business case, the value proposition and the national sustainability best practices and innovations
- Work with key New Jersey stakeholders to develop and present the business case to payers, including the state Medicaid agency
- Represent the Centering Healthcare Institute at national and local conferences, stakeholder meetings, and in discussion with potential partners
Experience, Skills, and Attributes
- Minimum five to seven years health system experience or comparable
- Bachelor’s degree preferred
- Strong understanding of and experience in change management within in a healthcare practice setting
- Passionate about improving the U.S healthcare delivery system, in particular the patient experience of healthcare, health outcomes, and health equity
- Familiarity with healthcare payment infrastructure and quality-based reimbursement a plus
- Highly motivated self-starter with an entrepreneurial spirit
- Strong communicator skilled at presenting ideas and concepts
- Comfortable engaging with clinic and community leaders, physicians, government agency heads
- Strong organizational skills, including the ability to manage multiple projects at once, respond to tight timelines, pay attention to detail, and operate effectively in a fast-paced environment
- Ability to work with diverse groups of people
- Access to transportation
- Bilingual/bicultural background highly desirable
- Work on projects to create, refine, and evaluate products, resources, and services that best meet the clients needs
- Collaborate intra- and cross-departmentally to identify opportunities for improvement;
- Provide timely, accurate, and reliable communication including activity and productivity reports, expense reports, and responses to internal and external customers
- Comfortable using and learning new technologies that support communication, data collection and reporting

Compensation and Benefits
